Social Worker

About the role

The Medical Social Worker will provide rotating onsite social work coverage across three hotel-based client service locations (Syosset, Planview and Woodbury).

Responsibilities

• Assisting clients with SSI/SSD applications
• Supporting Medicaid enrollment and recertification
• Aiding with SNAP applications and other public assistance programs
• Connecting clients with food assistance programs and transportation resources
• Providing guidance and support with housing applications
• Aiding in transition planning to permanent housing
• Collaborating closely with DSS case managers and other community partners
• Coordinating discharge planning and hospital transitions
• Delivering crisis intervention and immediate support services
• Linking clients to appropriate community-based resources
• Advocating for client needs and ensuring access to appropriate support services

Requirements

• Active New York State LMSW or LCSW license
• Master’s Degree in Social Work (MSW)
• 1+ year of experience in healthcare or social services (home care preferred)
• Strong case management and care coordination skills
• Excellent communication and interpersonal skills
• Ability to manage caseloads in a fast-paced environment
• Compassionate, patient-focused approach to care
