Social Worker

We are seeking a compassionate and experienced Social Worker to join our long-term care team at our skilled nursing facility in Providence, RI. You will have the opportunity to make a meaningful difference in the lives of our residents and their families. Responsibilities

• Assists residents and their families in connecting with community resources, healthcare providers, and other social services
• Conducts assessments and evaluations to identify psychological, financial, religious, and physical needs
• Develops and implements individualized care plans that meet the unique needs of each resident
• Collaborates with other disciplines to ensure comprehensive care and support
• Provides counseling and support services to residents and their families
• Maintains accurate and detailed records of resident interactions and services provided

Requirements

• Bachelor's of Science in Social Work or a related field
• Minimum 2 years of experience as a Social Worker
• License: LSW, LCSW or LICSW preferred but not required

Skills

• Strong communication, assessment, and social work methods and techniques
• Ability to work effectively with a variety of disciplines
• Knowledge of social systems and ability to implement pertinent state and federal regulations
