Serenity Hospice is seeking an MSW for our Portland Metro team. We help families through their toughest moments and support our clinicians every step of the way, prioritizing connection over quotas to build trust and truly focus on care.
Schedule
Monday-Friday, 8am to 5pm

Responsibilities
- Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care
- Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
- Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers; assists team members as needed to ensure consistent service
- Counsels patient and family/caregivers and staff as needed in relationship to stress and other identified coping difficulties
- Provides accurate and timely documentation of patient services to reflect the plan of care
- Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver
- Obtains resources from community social, health, pharmaceutical, and welfare agencies to assist in meeting patient/family needs
- Serves as liaison between patients and families/caregivers and community agencies
Requirements
- Social Work Degree:
- Idaho: Bachelor’s degree in Social Work; Master’s degree preferred
- Illinois: Bachelor’s degree in Social Work; Master’s degree preferred
- New Mexico: Master’s Degree in Social Work required
- Ohio: Bachelor’s degree in Social Work; Master’s degree preferred
- Oregon: Master’s Degree in Social Work required
- Pennsylvania: Bachelor’s degree in psychology, sociology, or other field related to social work; Master’s degree preferred (must be supervised by a full-time MSW and have at least 1 year of social work experience in a healthcare setting)
- Tennessee: Master's Degree in Social Work required
- Texas: Bachelor’s degree in Social Work; Master’s degree preferred
- Virginia: Bachelor’s degree in Social Work; Master’s degree preferred
- The bachelor’s degree can be in Psychology, sociology, or other field related to Social Work
- Current State Licensure in Social Work:
- Idaho: LSW, LMSW, or LCSW
- Illinois: LSW or LCSW
- New Mexico: LMSW or LCSW
- Ohio: LSW, LISW, or LPSW-S
- Oregon: Licensed not required
- Pennsylvania: LSW or LCSW, preferred
- Tennessee: LMSW, LCSW, or LAPSW (can practice under the supervision of an LCSW)
- Texas: LBSW, LMSW, or LCSW
- Virginia: Licensed not required
- Minimum of one year’s experience in health care or hospice setting
- Valid driver’s license and proof of insurance is required
