About the role

BronxWorks helps individuals and families improve their economic and social well-being through programs in workforce development, children/youth/families, seniors, and homeless prevention. The Brook Supportive Housing Program permanently houses 189 formerly street-homeless residents, most with serious mental and medical conditions (including HIV/AIDS) and substance misuse challenges. Social Workers join a multidisciplinary team to help residents transition into permanent housing with wraparound supports.

Responsibilities

• Maintain a caseload of 14–18 individuals with chronic homelessness history
• Monitor and document client progress toward treatment plan goals
• Conduct regular case conferences with clients and service providers
• Perform monthly home visits and fieldwork with clients
• Assist with managing and directing walk-in clients
• Document progress notes for all encounters and services provided
• Participate in regular meetings, trainings, and weekly supervision
• Provide intensive case management, crisis counseling, and substance abuse counseling
• Provide group counseling as assigned
• Opportunities to earn LCSW and LMHC qualifying licensing hours

Program-specific responsibilities

• Engage, intake, assess, develop service plans, refer, advocate, provide therapeutic counseling, reassess, and write exit summaries
• Monitor and document client progress toward service plan goals; record all interactions
• Teach parenting classes as required
• Maintain familiarity with on- and off-site program resources
• Coordinate client services with mental health, medical, employment, educational, childcare, and other providers
• Review and sign off on service plans and psychosocial assessments
• Conduct regular home and field visits
• Conduct regular case conferences with clients and providers
• Assist with special projects and represent the agency at inter-agency events
• Act as program contact for client-related inquiries
• Supervise paraprofessional staff and cover office functions in supervisor’s absence
• Complete program reports and perform additional duties as assigned

Requirements

• Master’s degree in social work
• New York State LMSW certification required, or ability to obtain within six months of hire
• Proficiency in English; proficiency in Spanish or another second language preferred
• Strong oral and written communication, time management, and organizational skills
• Proficiency in Microsoft Office suite and standard business technology

• Ability to climb five flights of stairs for home visits/fieldwork
• Ability to bend and retrieve objects/documents
• Ability to travel throughout New York City and adjacent counties via public transportation; travel up to 1 mile in all weather conditions

Preferred qualifications

• Spanish bilingual preferred
• Demonstrated ability to work from a strengths-based perspective, overcoming stigma and navigating complex systems
• Adaptability to an active environment, exceptional time management, and strong attention to detail

Work environment

Some evenings and weekends required. Opportunities for group work and LCSW qualifying hours. The Brook provides high-level support via weekly individual supervision, rounds, on-site trainings, and staff meetings, and supports continuing education and career growth.
