About the role

Assessing individuals, developing individual care plans and determining necessary resources. Advocating for and assisting individuals and families in accessing and obtaining services. Contacting service providers and attending interdisciplinary conferences to determine appropriate placement and referrals. Monitoring individual rehabilitation goals and progress. Providing crisis intervention services.

Requirements

• Bachelor's in Social Work degree, working under the supervision of a licensed master social worker (LMSW) or licensed clinical social worker or New York State LCSW or LCSWs
• Valid driver's license

Schedule

• Monday through Friday, 8:00am - 4:30pm
• Anticipated to last 6 months, but is not guaranteed

Additional details

• Travel is required; individual will be provided with a state vehicle to use
• OMH required training will be provided by OMH
