Social Worker 2 - LMSW

Responsibilities

• Serves as the case manager for assigned patients.
• Counsels patients and families on living wills and durable power of attorney.
• Coordinates the interdisciplinary assessment process.
• Completes psychosocial assessments and conducts group and individual therapy.
• Develops and implements patient treatment plans based on thorough assessments.
• Documents patient interactions, activities, and treatment plans.
• Participates in treatment team conferences and follows established practice guidelines.
• Works collaboratively to improve processes and develop services.
• Provides counseling to staff members and facilitates support groups.
• Assists in the development and implementation of the interdisciplinary treatment plan.
• Acts as a patient advocate and promotes a therapeutic milieu.
• Engages in professional development and lifelong learning.
• Performs other duties as assigned or requested.

Qualifications

• Master's Degree in Social Work (LMSW) certification by the Louisiana State Board of Social Work Examiners.
