About the role

The Social Work Care Manager II – IPCM provides comprehensive care coordination and support services to individuals and families facing complex social and health-related challenges. This position works collaboratively with clients, their families, healthcare professionals, and community resources to address their unique needs and improve their overall well-being.

Responsibilities include conducting thorough assessments of clients' social, emotional, and environmental circumstances, developing personalized care plans, and coordinating access to necessary services and resources. Advocacy for clients' rights and empowerment is central, ensuring patients receive equitable and quality care. The role also involves crisis intervention, counseling, advocacy, and promoting preventive measures and education within the community.

Compassion, empathy, and strong communication skills are essential in building trusting relationships and supporting clients in achieving their goals for health and social stability. This role performs duties in a manner consistent with the Mercy mission, values, and service standards.

Requirements

  • Master's in Social Work (MSW), or completion of all MSW requirements awaiting conferment (for hires after 12/25/2011).
  • Current Social Work license in the state of employment (LMSW, LCSW).
  • BLS (Basic Life Support) certification at hire or within 90 days.

Qualifications

  • Preferred: Certification in Case Management.
  • Preferred: 2 years’ experience in an acute care hospital setting.
