Responsibilities

• Provide mental health services to patients, including assessments, counseling, and case management
• Collaborate with interdisciplinary teams to develop and implement personalized care plans
• Identify and address social determinants of health that impact patient outcomes
• Utilize evidence-based practices to deliver high-quality patient care
• Maintain accurate and timely documentation of patient interactions and services rendered
• Participate in quality improvement initiatives to enhance patient outcomes and reduce hospitalizations

Requirements

• Master's degree in Social Work (MSW) or related field
• Licensure as a Licensed Clinical Social Worker (LCSW) or eligibility for licensure
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment with multiple priorities
• Familiarity with electronic health records and practice management systems
