Social Work Assistant

Join our Social Work team as a Social Work Assistant and play a key role in supporting residents and families during important moments in their lives. This role is perfect for someone who is compassionate, organized, and energized by building relationships.

Responsibilities

• Help support residents’ social, emotional, and spiritual well-being
• Aid with admissions and discharges, assisting residents and families with transitions
• Provide emotional support and crisis assistance when needed
• Act as a connector between residents, families, care teams, and community resources
• Participate in care planning meetings and collaborate with an interdisciplinary team
• Aid with documentation, including social services sections of required records
• Build and maintain relationships with local and state community agencies

Requirements

• A high school diploma or equivalent
• (Or willingness to complete) Nebraska Health Care Association Social Service Designee training
• A warm, compassionate personality and strong people skills
• The ability to communicate clearly, maintain confidentiality, and stay organized
• Basic computer skills and comfort with documentation
• A genuine interest in working with older adults
