Social Work Assistant

About the role

The Central Durham Social Work Hub is recruiting a Social Work Assistant. Durham County Council (DCC) and Tees Esk and Wear Valley Foundation Trust (TEWV) are integrated under a partnership agreement to deliver health and social care services to people with complex mental health problems in County Durham. The team includes staff from Adult Care Services (DCC) and health staff (TEWV). This is an exciting opportunity for a motivated individual interested in supporting the delivery of good quality mental health care to adults across the county.

Responsibilities

• Provide assessment, care planning, treatment and interventions for service users/patients with social care needs.
• Work alongside social work and health colleagues to support individuals in the community
