Social Services Specialist II

Responsibilities

Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community.

Provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning.

Knows about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs.

Demonstrates skills in planning, organizing, and managing multiple functions and complex processes.

Requirements

• Location: Winter Haven Hospital - Winter Haven, Fl
• Status: Part time, 16 hours per week
• Schedule: 2 shifts per week, weekdays and occasional weekends
• On Call: no

Qualifications

• Preferred ACM (Case Management) or Preferred CCM (Case Manager)
• Education Required: Masters in Social Work, Sociology, or Related Field
• Strongly Preferred: Masters Degree in Social Work
• Experience Required: 6 months Internship or Required 1 year Social Work in an acute care or hospital setting
