Responsibilities
- Aids in interviewing and obtaining information from residents and families to develop psychosocial histories, assessments, and care plans upon admission, quarterly, and upon changes in condition.
- Aids in the transition and adjustment of residents and families to the facility, and assists with determining legal status and implementing resident and family wishes regarding treatment.
- Informs residents and families of their personal and property rights, and provides input on social, psychological, and emotional needs.
- Arranges residents' appointments including transportation, and assists in finding and utilizing community resources.
- Participates in resident and family council meetings, and develops a working knowledge of relevant regulations and reimbursement processes.
- Supports resident theft and loss prevention programs, and works with the Ombudsperson to address resident rights questions or concerns.
- Aids in discharge planning services, including arranging for follow-up services, transfers, and post-discharge plans, and provides orientation for residents and families regarding discharge.
Qualifications
- Previous experience with geriatric population or in long-term care settings is preferred.
- A high school diploma or equivalent is required; an associate or bachelor's degree is preferred.

- The ability to interact tactfully, effectively, and professionally with various stakeholders, including employees, residents, family members, visitors, government agencies, and the general public.
- The ability to relate to and work with physically and/or mentally ill, disabled, elderly, emotionally upset, and at times hostile people within the facility, treating them with respect and consideration regardless of their cognitive or functional level.
