Social Services Director

About the Role

A Social Services Director working with our Skilled Nursing facility will counsel and aid residents and families requiring assistance. This position meets those needs through individual and group social services programming, promotes residents’ potential during their stay, and collaborates with physicians and residents in preparation for discharge.

Responsibilities

• Interviews all new residents to assess their psychosocial needs
• Performs admission and discharge planning coordination
• Counsels residents individually or with family/support groups
• Refers residents to needed services such as Dental, Optometry, Audiology, Podiatry, Psychology, Psychiatry, and other community health resources
• Compiles records and prepares reports such as theft and loss
• Coordinates with the nursing department to meet residents’ optical, dental, and audiological needs and arranges transportation
• Prepares timely documentation of social service interventions and assessments in the resident's chart
• Attends resident care meetings, rehabilitation and utilization review, quality assurance, and department head meetings
• Completes appropriate sections of MDS timely
• Other duties as assigned

Requirements

• Bachelor's Degree in Social Services or similar field
• Skilled Nursing experience preferred

Schedule

Full-time, Monday-Friday (Weekends as needed)
