About the role

As a Social Services Assistant, you will support the social service function in a long-term care and short-term rehabilitation nursing center. You will work closely with the Social Services Manager to:

  • Welcome and admit new residents and patients
  • Complete documents related to psychosocial assessments, care plans, and discharge plans
  • Ensure a comfortable and overall positive experience is achieved while completing treatment plans

Responsibilities

  • Support the Social Services Manager in admitting new residents and patients
  • Complete psychosocial assessments and care plans
  • Work on discharge plans to ensure a smooth transition
  • Ensure a positive experience for all residents and patients

Requirements

  • Minimum high school diploma or equivalent
  • Minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology
  • One year supervised social work experience working directly with individuals in a health care setting
  • One or more years' experience in health care education/ marketing / insurance or commensurate educations preferred
  • Familiarity with medical terminology
  • Proficiency in using personal computers

Qualifications
