Position Summary

The Social Services Assistant works closely with the Director of Social Services to provide emotional, social, and psychosocial support for our residents while ensuring compliance with all state, federal, and facility regulations. This position plays a vital role in enhancing residents' quality of life and supporting successful admissions, transitions, and discharges.

What You'll Do

  • Aid with resident assessments, care planning, and required documentation
  • Support residents and families during admissions, transfers, and discharge planning
  • Help residents adjust to long-term care and promote a positive living environment
  • Advocate for residents' rights, dignity, and overall well-being
  • Serve as a liaison between residents, families, and the interdisciplinary care team
  • Afford coordination of resident-centered programs and activities that promote emotional and social wellness
  • Maintain accurate, timely, and confidential documentation
  • Help connect residents and families with appropriate community resources as needed
  • Support compliance with state, federal, and company regulations

What We're Looking For

  • A high school diploma or equivalent
  • An associate's or bachelor's degree in Social Work, Psychology, Human Services, or a related field
  • Previous experience in skilled nursing, long-term care, healthcare, or social services
  • Excellent communication, organizational, and interpersonal skills
  • Strong problem-solving abilities and attention to detail
  • A compassionate, empathetic, and resident-focused mindset
  • Ability to maintain confidentiality and professionalism in all interactions
