Social Services Assistant

Pay

Full-time position. Hourly wage range: $18.00 – $20.00. The wage range takes into account factors including skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.

Responsibilities

• Complete the initial Social Service Assessment and the initial patient interview profile to begin discharge planning upon admission
• Provide information to family and/or patients/residents relating to Medicare, Medicaid, and other financial assistance programs available to the patient/resident
• Work closely with Case Management to coordinate patient discharges

Requirements

• High School Diploma or equivalent G.E.D. required
• Social Work License preferred, not required
• Experience in long term care or skilled nursing facility required
• Reliable transportation required
