Duties
- Conducting resident interviews to gather information about their social, emotional, and financial needs and concerns.
- Assisting in discharge planning by coordinating with healthcare providers, families, and community resources to ensure a smooth transition for residents.
- Documenting interviews, phone calls, and care conferences on a daily basis in accordance with facility policies and procedures.
- Maintaining regular communication with interdisciplinary team members, including nurses, therapists, and social workers, to ensure effective care planning and coordination.
- Adapting your daily schedule to accommodate the needs of staff, residents, and their families.
- Provide Social Services oversight completing duties as assigned for the residents of Eden Senior Care.
- Work closely with Marketing Director to coordinate referral-to-admission process.
- Communicate with insurance companies to obtain prior authorization for admissions; keep record of insurance updates and communicate with departments included in update to ensure it is done timely.
- Aid new residents and family members with completion of Admission Paperwork.
- Provide information to residents/families in regards to Medicare, Medicaid and other financial assistance programs available to the resident; assist with completion of Medicaid applications as deemed necessary.
- Ensure PASRR is completed to state requirements.
- Participate in resident assessment - provide background information and social history, as identified in the MDS process, in accordance with the RAI manual.
- Involvement in planning objective and goals for the resident; develop a written plan of care to work towards meeting identified goals.
- Attend and lead Behavior Meeting and monitor Gradual Dose Reductions (GDR) and consents for psychotropic medications according to state regulations.
- Complete timely progress notes on each resident.
- Have an open-door policy to all residents, family members, and staff to listen to their problems or needs; do effective counseling or referrals to meet their needs.
- Report concerns/grievances to Executive Director and initiate investigation.
- Serve as an advocate for the resident and his/her rights, helping the resident learn assertiveness in communicating their needs, wants, and problems to the appropriate departments and agencies.
- Participate in Monthly QAPI Committee meetings.
- Attend Stand-Up meeting daily; report to IDT about pay changes, care conferences, concerns, etc.
- Initiate and lead the discharge planning process; coordinate with outside resources such as: DME, Home Health, Hospice, etc. to ensure a safe transition to home or lower care setting.
- Issue denial letters according to state regulation.
- Aid in providing in-service education and orientation for employees regarding Resident Rights and social service needs of residents.
Requirements
- Preferred: Bachelor's degree in Social Work or a Bachelor's degree in a Human Services field including but not limited to Social Services, Sociology, Special Educations, Rehabilitation Counseling, and Psychology.
- Preferred: Licensed Social Work certificate in the state of Pennsylvania, current and valid.
- Preferred: One year of supervised social work experience in a health care setting working directly with individuals.
- Able to promote cooperation between social services and other department employees.
- Must be able to be professional over the phone and in person when representing the company.
- Able to take pride in his/her performance.
- Able to write and give clear, concise instructions and remain calm during emergency situation.
