Social Services Assistant

About the role

The Social Services Assistant works under the supervision of the Social Services Director to coordinate patient support and discharge.

Responsibilities

• Complete the initial Social Service Assessment and the initial patient interview profile to begin discharge planning upon admission.
• Provide information to family and/or patients/residents regarding Medicare, Medicaid, and other financial assistance programs available to the patient/resident.
• Work closely with Case Management to coordinate patient discharges.

Requirements

• Minimum of a High School Diploma or equivalent G.E.D.
• Social Work License preferred, not required.
• Experience in long‑term care or skilled nursing facility.
• Reliable transportation required.

Pay

USD $25.00 – USD $31.00 per hour. The wage range reflects factors such as skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.
