About the role

Help connect people to care when it matters most. As a Social Service Worker on the Access Team at Valley Behavioral Health, you will support individuals seeking services through the walk-in clinic by completing intake assessments and ASAM evaluations for Adult Outpatient mental health, behavioral health, and substance use treatment programs. Working collaboratively with multidisciplinary teams and community partners, this role ensures individuals are connected to appropriate care while providing compassionate, client-centered support throughout the intake process.

Responsibilities

  • Conduct intake assessments and ASAMs for incoming clients with our Adult Outpatient programs (mental health, substance use disorder, court-ordered).
  • Direct clients’ care by developing, implementing, and managing personalized care plans with specific treatment goals and objectives for each client.
  • Follow agency documentation policies for all phases of treatment, including assessments, care plans, treatment transitions, discharge plans, and client care.
  • Review and assess care plans.
  • Coordinate services among internal and external agencies and providers.
  • Ensure continuity of care.
  • Meet required productivity standards and other organizational expectations.

Requirements

  • Bachelor’s degree in Social Work or related field.
  • Internship experience as required for SSW license.
  • SSW licensure in state of practice.
  • CPR certification (post-hire as needed).

Preferred Qualifications

  • Previous experience with behavioral health.

Pay

$23.00–$25.88/hour (range starts at $23.00/hour; pay is calculated based on years of licensed working experience).

Schedule

Monday–Friday | 8:00 AM–4:30 PM
