Social Service Director

About the role

The Social Services Director / long-term care Social Worker is responsible for assessing, advocating, and planning for the psychosocial needs of patients across various age groups. Expertise in crisis intervention, mandated reporting, and care coordination for those at risk is required.

Responsibilities

• Provide assessment, advocacy, and discharge planning for patients' psychosocial needs
• Coordinate the psychosocial needs of residents and families
• Work closely with the Nursing Department to support resident care
• Support education and advocacy for patients, families, and significant others

Requirements

• Bachelor’s Degree in Social Services
• CSW (Certified Services Worker) certification—State of Missouri
• Knowledge of community resources
• Experience in healthcare social work
• Strong interpersonal and communication skills
• Professional demeanor with patients, families, interdisciplinary teams, and outside agencies
• 2–5 years of acute care or long-term care experience preferred
• MDS experience preferred
• Long-term care social work experience a plus
