About the Role

This position is primarily responsible for coordinating home and community-based services to individuals and families within agency policies, coordinating social programming for the property, and providing social service intervention as needed.

Responsibilities

  • Assess residents who may require assistance in daily living activities, a higher level of care, or are in transition to a higher level of care. Involve family and other professionals as appropriate.
  • Make referrals to community service agencies and follow up on services.
  • Coordinate with hospital staff regarding discharge planning for residents returning to their apartments.
  • Provide crisis intervention with residents on a short-term basis as needed.
  • Provide case management services for residents receiving services from various community agencies.
  • Advocate for residents and maintain a current knowledge base of services and programs impacting seniors and/or disabled individuals to enhance assistance in securing support.
  • Identify education and health programs to enhance residents’ quality of life.
  • Establish and maintain contacts with community agencies offering services to residents; develop and update a resource directory.
  • Build informal support systems for residents with family members, other residents, and friends as needed.
  • Publish and distribute a building newsletter.
  • Fundraise to cover the cost of programs and activities in the building.
  • Demonstrate sensitivity and responsiveness to cultural differences within the resident population.
  • Maintain resident confidentiality.
  • Use social services software to maintain resident files in compliance with HUD requirements.
  • Report instances where there may be a question of abuse or neglect and take proactive steps to protect residents.
  • Participate in overall planning for activities at the property.
  • Serve as a resource to the Manager related to on-going policy development and social planning within the building.
  • Communicate and coordinate with property management staff regarding program and resident issues.
  • Participate in professional organizations and training opportunities to meet HUD requirements for continuing education units (CEUs).
  • Secure needed continuing education credits to meet HUD requirements and attend HUD training as necessary.
  • Be punctual and timely in meeting all performance requirements, including attendance standards and work deadlines.

Requirements

  • High school diploma or general education degree (GED); minimum of two years related experience and/or training; or equivalent combination of education and experience.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups of customers or employees.
  • Knowledge of spreadsheet software (Excel), word processing software (Word), electronic mail software (Outlook), and presentation software (PowerPoint).
  • Bachelor’s Degree or Associate’s Degree in Social Work, Counseling, Sociology, or other social science major (desired).
  • Previous experience as a Social Service Coordinator (desired).
  • Access to reliable transportation.
  • Ability to adapt to changes in business practices with minimal or no advance notice.

    • Oral: Speaks clearly and persuasively in positive or negative situations; listens and gets clarification; responds well to questions; demonstrates group presentation skills; participates in meetings.
    • Written: Writes clearly and informatively; edits work for spelling and grammar; varies writing style to meet needs; presents numerical data effectively; able to read and interpret written information.
  • Understanding of social service funding, eligibility, and access to community services.
  • Understanding of the neighborhood and available services.
  • Excellent interpersonal relationship skills.
  • Experience with social service software.
