Social Rehab Counselor
Position Overview
Under the direct supervision of the Team Leader, the Social Rehab Counselor (SRC) provides both clinical and residential support to individuals served.
Responsibilities
- Provide community support, including:
- Transportation to day programs, medical, benefits, or employment appointments
- Participation in leisure and goal-related activities
- Aid with grocery shopping and errands
- Deliver case management services such as:
- Support with medical benefits
- Cordination with therapists and treatment teams
- Support individuals with:
- Medication management
- Daily living skills (meal preparation, laundry, home cleanliness)
- Maintain accurate and timely service documentation and Medicaid billing
- Participate in house, staff, and mandatory monthly MI supervision meetings
- Collaborate with multidisciplinary treatment teams, including PSR and Supported Employment staff
- Actively participate in case staffing as needed
- Communicate any concerns or issues to the Team Leader promptly
- Meet or exceed established service billing benchmarks

- Strong decision-making and problem-solving abilities
- Effective interpersonal and communication skills (oral and written)
- Ability to collaborate with diverse stakeholders including staff, families, and community agencies
- Capable of planning, organizing, and executing program goals and procedures
- Self-motivated and able to work independently
- Professional demeanor that supports a therapeutic environment
Physical Demands
- Lift over 40 pounds from floor to waist unassisted
- Push and pull up to 75 pounds; push wheelchairs up to 200 pounds
- Perform physical movements such as squatting, stooping, crouching, and kneeling
- Stand, sit, or remain stationary for prolonged periods
- Drive and operate motor vehicles
- Engage in repetitive motions using hands and fingers for computer and phone use
Qualifications
- Bachelor’s Degree in Social Work, Psychology, or related field
