Skilled Nursing Facility Per Diem - Nurse Practitioner or Physician Assistant

Responsibilities

• Deliver compassionate care in Geer Assisted Living with potential to expand.
• Work within a multi-disciplinary care team in the Skilled Nursing Facilities (SNF) setting in the Bloomfield, CT area.
• Provide routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients.
• In consultation with the physician(s) and care team members, develop plans for further evaluation of health needs, including referrals to other health team members or specialists.
• Treat problems within the scope of the provider’s practice as designated by protocols and refer appropriately to physicians if needed.

Requirements

• Licensure: Unrestricted State of Connecticut Medical License (or eligible)
• Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA
• Experience: At least two years of post-graduate experience as a Nurse Practitioners (APRN) or Physician Assistants (PA), preferably in the Skilled Nursing Facility and Geriatrics setting
