About the role

Signify Health, part of CVS Health®, is seeking a Full-Time Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. In this role, you’ll be primarily traveling—up to 100% of the time—across a defined coverage area, as defined by business needs. This may require overnight hotel stays, car rentals, and/or flights, depending on the prescribed coverage area. Holding multiple state licenses will be required, as dictated by business needs.

You’ll play a critical role in building trusted relationships to make people healthier, bringing flexible, comprehensive, and personalized health evaluations right to people’s front doors.

Responsibilities

• Conduct 45-minute in-home health evaluations, including a medication and medical history review, a physical evaluation, and additional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the person’s health plan.
• Use an iPad and an intuitive clinical workflow for each evaluation; no prescribing, lab ordering, or altering of current treatment regimens is required.
• Join a national network of purpose-driven clinicians dedicated to improving health outcomes.
• Visit millions of people nationwide to connect them with the right care for their unique needs.
• Fulfill the requirements of the specified role as a clinician colleague with Signify Health.
• Conduct virtual visits as needed.
• Provide all Signify Health services, including Diagnostic & Preventive Services products (e.g., spirometry, diabetic eye exams, bone density screening).
• Be flexible to travel locally and within licensed states.

Work Environment

As an in-home clinician, your primary workplace is the private residence of health plan members. These environments are diverse and may present a variety of conditions, including household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to comfortably and safely perform their duties in these settings.

Qualifications

• Active, unrestricted medical license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage areas, as required.

PA Coverage Markets Include: Allentown, Altoona, Bradford, Erie, Harrisburg, Johnstown, Lancaster, Philadelphia, Pittsburgh, Scranton, State College, Sunbury, and Williamsport.

Our concierge-style Network Activation team will support you in obtaining, renewing, enrolling, or verifying additional state licensure in new states as business needs arise.

Schedule

• Full-time position.
• Anticipated weekly hours: 40.

Pay

The typical pay range for this role is $174,070.00 - $374,920.00. This pay range represents the base annual full-time salary. The actual base salary offer will depend on a variety of factors including experience, education, geography, and other relevant factors.
