About the role

Signify Health, part of CVS Health®, is seeking a Full-Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. In this role, you’ll primarily travel—up to 100% of the time—across a defined coverage area, which may require overnight hotel stays, car rentals, and/or flights. Holding multiple state licenses will be required as dictated by business needs.

You’ll play a critical role in building trusted relationships to make people healthier, bringing flexible, comprehensive, and personalized health evaluations directly to their front doors.

Responsibilities

• Conduct 45-minute in-home visits, including a medication and medical history review, physical evaluation, and additional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the person’s health plan.
• Use an iPad and intuitive clinical workflow for evaluations; no prescribing, lab ordering, or altering current treatment regimens required.
• Travel locally and within licensed states, with flexibility for overnight stays and virtual visits as needed.
• Provide all Signify Health services, including Diagnostic & Preventive Services (e.g., spirometry, diabetic eye exams, bone density screening).
• Work in diverse home environments, which may include pets, secondhand smoke, or other airborne allergens.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Able to obtain and maintain multi-state licenses in coverage areas as required.
• Able to cover all counties within a state and flex to other areas based on business need.

PA Coverage Markets Include: Allentown, Altoona, Bradford, Erie, Harrisburg, Johnstown, Lancaster, Philadelphia, Pittsburgh, Scranton, State College, Sunbury, and Williamsport.

Our concierge-style Network Activation team will support you in obtaining, renewing, enrolling, or verifying additional state licensure as needed.

Pay

The typical pay range for this role is $87,035.00 - $187,460.00 annually. The actual base salary offer will depend on factors including experience, education, geography, and other relevant considerations.

Schedule

• Full-time position.
• Anticipated weekly hours: 40.
