Job Summary
The Shelter Community Support Provider will provide intensive, clinical case management to consumers with serious mental illness needing such support, or who have been hard to engage in the mental health system. Services will be provided at shelter or in an outreach setting.
Starting Salary
Starting Salary: $38,000-$44,000/year

About the Role
This position requires regular contact with consumers, family members, and other members of the "natural helping network."
Responsibilities
- Provide skills training and assistance in areas of daily living activities including transportation, money management, cleaning, laundry, food preparation, personal grooming, and health care
- Assist with advocacy needs; facilitate "mediation" as appropriate
- Provide counseling and intensive support to consumers during psychiatric crises or periods of transition
- Assist consumer with identifying and obtaining entitlements, which meet consumer's need
- Provide assistance in developing and maintaining a "natural support network" of family and friends
- Assist consumer in maintaining the appropriate upkeep of apartment or shelter space that will ensure healthful and safe living
- Coordinate development of meaningful day activities, including employment or volunteer activities
- Assist with finding or developing recreational and social activities of the consumer's choice
- Maintain appropriate contacts with family members, others in the "natural helping network," outpatient therapists, and others as necessary to support consumers
- Provide transportation for consumers as appropriate, following program guidelines
Qualifications
- Bachelor's or Associate's degree in a mental health discipline or high school diploma and five years' related experience
- Valid driver's license and reliable transportation
- Bilingual candidates are strongly encouraged to apply

- Ability to work independently and as part of a team
- Knowledge of mental health disorders and treatment options
- Experience working with individuals with serious mental illness
- Ability to manage multiple tasks and prioritize responsibilities
