Summary Job Description
The CFTSS (Children and Family Treatment and Support Services) and HCBS (Home and Community Based Services) Service Provider is responsible for delivering critical behavioral health services to children, youth, and families. These services include Community Psychiatric Support (CPST), Psychosocial Rehabilitation (PSR), Caregiver Family Supports, Prevocational Services, Supportive Employment Services, and Community Self-Advocacy Training. Services are provided in home or community settings and during family visitations, utilizing visit coaching principles to ensure effective support.
Responsibilities
- Participate in the development and ongoing monitoring of the child’s Treatment Plan.
- Provide direct services to children and families on caseload, primarily throughout Long Island.
- Document all service provision and contacts.
- Attend necessary meetings and coordinate care with other providers and collaterals to support optimal functioning within the community.
- Provide CPST and PSR during family visitations, incorporating visit coaching best practices.
- Attend all required trainings.
Requirements
- Bachelor’s degree in social work, psychology, or a related field, plus four years of related experience.
- Master’s degree in social work, psychology, or a related field preferred, with one year of related experience.
- Excellent writing, communication, and word processing skills.
- Ability to travel to various communities and work a flexible schedule, including weekends and evenings.
- Possession of a valid NYS driver’s license.
Qualifications
- Experience in providing behavioral health services to children and families.
- Knowledge of community-based services and visit coaching principles.
- Strong organizational and documentation skills.
Pay
This is an hourly position. Salary will commensurate with experience.
