About the role
A Service Facilitator supports individuals with mental health challenges through care coordination, skill development, and treatment planning within a community-based setting.
Responsibilities
- Conduct assessments
- Develop and implement personalized service plans
- Facilitate recovery-focused team meetings
- Connect clients to community resources
- Maintain accurate documentation
- Collaborate with treatment teams, providers, and support systems to promote client independence and well-being
Qualifications
- Master's degree in human services or related field
- Valid driver's license
- Ability to travel within the community
- Flexible hours
Preferred experience in mental health, case management, or community services, especially with children and families. Relevant degrees include social work, psychology, counseling, and related fields.
