About the role
The Service Facilitator partners with individuals, families, and community providers to assess needs, develop personalized, person-centered service plans, and coordinate services that promote independence and well-being.
Responsibilities
- Conduct functional assessments
- Facilitate team meetings
- Connect clients with community resources
- Maintain accurate documentation
- Advocate for positive outcomes using trauma-informed, strengths-based approaches
Requirements
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field
- Experience working with vulnerable populations preferred
- Valid driver’s license and reliable transportation for community travel
