About the role
We are seeking a compassionate Service Facilitator for a community-based mental health program.
Responsibilities
- Conduct assessments and develop individualized service plans in collaboration with clients, families, and providers.
- Coordinate and authorize community-based services and resources.
- Facilitate recovery-focused team meetings and support client independence.
- Maintain accurate documentation and collaborate with treatment teams and community partners.
Qualifications
- Master's degree in human services-related fields such as Social Work, Psychology, Counseling, or related disciplines.
- Valid driver's license and reliable transportation for community travel.
- Ability to work flexible hours, including occasional evenings.
- Preferred experience in mental health, behavioral health, case management, or community services, especially with diverse populations and trauma-informed, recovery-oriented approaches.
