About the role
The Service Facilitator plays a vital role in the Comprehensive Community Services (CCS) Program, partnering with individuals, families, and community providers to assess needs, develop personalized service plans, and coordinate resources to promote independence and well-being.
Responsibilities
- Conducting functional assessments
- Creating person-centered plans
- Facilitating team meetings
- Maintaining accurate documentation
- Advocating for clients using trauma-informed, strengths-based approaches
Qualifications
- Bachelor’s degree in Social Work, Psychology, or related fields
- Preferred experience working with vulnerable populations

- Ability to manage multiple services independently
- Valid driver’s license and reliable transportation
