About the role
Seeking a compassionate Service Facilitator for the CCS Program, responsible for assessing client needs, developing personalized service plans, and coordinating community resources to promote independence and well-being.
Responsibilities
- Conducting functional and needs assessments
- Creating and implementing person-centered service plans
- Connecting clients with community resources and services
- Facilitating team meetings and collaborating with families, providers, and support networks
- Maintaining accurate documentation and client records
- Advocating for clients using trauma-informed and strengths-based approaches
Qualifications
- Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field
- Preferred experience working with individuals, families, or vulnerable populations
- Strong communication, organization, and relationship-building skills
- Ability to manage documentation, coordinate multiple services, and work independently
- Valid driver’s license, reliable transportation, and willingness to travel within the community
