Service Facilitator (CCS)

About the role

Seeking a compassionate Service Facilitator for the CCS Program, responsible for assessing client needs, developing personalized service plans, and coordinating community resources to promote independence and well-being.

Responsibilities

• Conducting functional and needs assessments
• Creating and implementing person-centered service plans
• Connecting clients with community resources and services
• Facilitating team meetings and collaborating with families, providers, and support networks
• Maintaining accurate documentation and client records
• Advocating for clients using trauma-informed and strengths-based approaches

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, Sociology, Child Development, or related field
• Preferred experience working with individuals, families, or vulnerable populations
• Strong communication, organization, and relationship-building skills
• Ability to manage documentation, coordinate multiple services, and work independently
• Valid driver’s license, reliable transportation, and willingness to travel within the community
