About the role

This role coordinates and facilitates community-based mental health services within a comprehensive program, emphasizing recovery and family-centered care.

Responsibilities

• Conduct assessments and develop individualized service plans.
• Connect clients to resources and facilitate team meetings focused on recovery and family-centered care.
• Document client records and collaborate with treatment teams, community providers, and support systems.

Requirements

• Master’s degree in a human services-related field.
• Valid driver’s license and the ability to travel locally.

• Preferred experience in mental health, therapy, case management, and working with diverse populations with complex needs.
