Responsibilities
- Conducting assessments and developing personalized plans
- Facilitating team meetings and coordinating services
- Maintaining accurate documentation and collaborating with treatment teams and community providers
- Connecting clients to support services and resources
- Supporting recovery-focused, trauma-informed, and person-centered care
Qualifications
- Master's degree in a human services-related field
- Valid driver's license and ability to travel within the community
- Experience with mental health, case management, or community services is preferred
