Service Facilitator (CCS)

About the role

We are seeking a Service Facilitator to join a community-based program supporting individuals with mental health challenges across all ages. In this role, you will coordinate services, develop personalized care plans, facilitate team meetings, and connect clients to community resources, promoting recovery and independence.

Responsibilities

• Collaborate with treatment teams, community providers, schools, and supports

Requirements

• Master’s degree in a human services field
• Valid driver’s license
• Ability to travel within the community

Qualifications

• Preferred experience working with diverse populations
• Preferred experience providing mental health or care coordination services
• Preferred familiarity with trauma-informed, recovery-oriented approaches

Pay

Compensation varies based on credentials, ranging from $27.20 to $30.20 per hour.
