Service Facilitator (CCS)

About the role

Seeking a dedicated Service Facilitator for a community-based mental health role supporting children, adolescents, and adults with mental health challenges.

Responsibilities

• Conduct assessments and develop personalized service plans in collaboration with clients, families, and providers.
• Coordinate and authorize community-based services and resources.
• Facilitate recovery-focused, family-centered team meetings.
• Maintain accurate documentation and complete reports.
• Collaborate with treatment teams, community agencies, schools, and support systems.

Qualifications and Skills

• Master's degree in human services or related field.
• Valid driver's license and reliable transportation to meet clients in community settings.
• Ability to work flexible hours, including occasional evenings.
• Preferred experience in mental health, behavioral health, therapy, case management, or community services.
• Experience with individuals and families with complex needs and recovery-oriented, trauma-informed care.
• Applicable degrees include Social Work, Psychology, Counseling, Marriage & Family Therapy, and related fields.
