About the role
We are seeking a Service Facilitator to support individuals facing mental health challenges through a person-centered, recovery-focused approach.
Responsibilities
- Conduct assessments and develop individualized service plans
- Coordinate and authorize community-based services
- Facilitate recovery-oriented team meetings
- Connect clients to resources
- Maintain accurate documentation
Qualifications
- Master's degree in a human services-related field
- Valid driver’s license
- Ability to travel within the community
Preferred experience
- Mental health or case management services
- Working with diverse populations with complex needs
- Trauma-informed, recovery-oriented practices
