Service Facilitator (CCS)

About the role

We are seeking a compassionate Service Facilitator to join a community-based mental health team.

Responsibilities

• Conduct assessments and develop individualized service plans in collaboration with clients and support systems.
• Coordinate, authorize, and facilitate recovery-focused, family-centered team meetings.
• Connect clients to community resources and document all interactions accurately.
• Collaborate with treatment teams, community providers, schools, and natural supports to ensure comprehensive care.

Qualifications

• A master's degree in a human services-related field such as social work, psychology, counseling, or related disciplines.
• Valid driver's license and reliable transportation with ability to travel within the community.
• Ability to work flexible hours, including occasional evenings.
• Preferred experience in mental health, behavioral health, case management, or community-based services, especially with children, families, or individuals with complex needs.
