Responsibilities
- Cook up and manage homecare services for NHTD clients, guaranteeing high-quality care and compliance.
- Act as the primary go-to person for clients, family members, and caregivers.
- Create, execute, and oversee individualized service plans.
- Earn out compliance with state regulations and program policies.
- Communicate effectively with healthcare professionals, service providers, and agencies.
- Maintain precise records and reports on client care and service coordination.
- Identify and resolve any service gaps to boost client support.
Requirements
- Prior experience in NHTD, TBI waiver programs, or homecare service coordination.
- Strong grasp of NYS DOH regulations and Medicaid-funded programs.
- Outstanding organizational and communication skills.
- Ability to work independently while coordinating with numerous stakeholders.
