• Experience in social service delivery with elderly and/or family populations

Responsibilities

• Serve as a liaison to community agencies
• Network with community service providers
• Seek out new services available to residents
• Identify low-cost service providers and/or negotiate discounts
• Monitor the ongoing provision of services from community agencies
• Keep documentation of the progress of the individual
• Manage the provision of supportive services where appropriate
• Engage all residents in the building(s) to identify areas of need
• Make referrals to community agencies when necessary
• Develop a Resource Directory
• Include a listing of state and/or local service providers
• Sponsor educational events
• Include subjects relating to health care, agency support, life skills, and referral sources

Qualifications

• A bachelor’s degree in social work or a related field is preferred
• Two or more years of experience in a social service delivery with elderly and/or family population
