Responsibilities
- Serve as a liaison to community agencies
- Network with community service providers
- Seek out new services available to residents
- Identify low-cost service providers and/or negotiate discounts
- Monitor the ongoing provision of services from community agencies
- Keep documentation of the progress of the individual
- Manage the provision of supportive services where appropriate
- Engage all residents in the building(s) to identify areas of need
- Make referrals to community agencies when necessary
- Develop a Resource Directory
- Include a listing of state and/or local service providers
- Sponsor educational events
- Include subjects relating to health care, agency support, life skills, and referral sources
Qualifications
- A bachelor’s degree in social work or a related field is preferred
- Two or more years of experience in a social service delivery with elderly and/or family population
