Service Coordinator - Full-Time - Days

Role Overview This position involves daily service coordination and care management for health plan members, focusing on assessment, planning, facilitation, and advocacy to meet their comprehensive health needs. Responsibilities include evaluating member needs, developing and monitoring care plans, coordinating with providers and families, providing education on healthcare procedures and benefits, and maintaining resource directories. Ensures compliance with regulatory standards and promotes quality improvement through health literacy and data-driven strategies. Acts as a liaison with providers, supports culturally inclusive practices, and maintains confidentiality and ethical standards. Qualifications require a BSN or MSW with 3+ years of behavioral health or care management experience. Valid licensure (RN or social work license) and preferably a Certified Case Manager (CCM) credential are desired. Proficiency in medical terminology, managed care standards, and Microsoft Office is essential.
