Service Coordinator

General Summary

The Service Coordinator is responsible for assisting and enabling children and families eligible for New Jersey Early Intervention Services System to receive the rights, procedural safeguards, and services that are authorized by the New Jersey Early Intervention Program. This individual coordinates services and advocates for families to assure linkage to all services provided within the Early Intervention Program and with other community resources in order to maximize their child’s abilities.

Essential Functions

• Contribute to The Cooperative’s commitment to equity and continuous quality improvement that aligns with the agency’s mission, goals, and advancement.
• Effectively cross-collaborate with agency staff, leaders, and board members to foster a comprehensive approach of information sharing, working committees, and collective representation.
• Routinely disseminate relevant information, presentations, training, and learning opportunities for internal program and agency staff.
• When applicable, will participate in relevant staff development opportunities (trainings, summits, conferences, etc.) to increase skill development, elevate program goals, and engage staff members with timely key learnings.
• Facilitates and participates in the development, review and evaluation of individualized family service plans with family input
• Captures and documents all service coordination activities and other reports in a timely, accurate and complete manner
• Maintains client and staff confidentiality
• Provides services in a culturally competent manner
• Demonstrates sensitivity, knowledge and an understanding of the challenges families with a child with disabilities may experience
• Performs other duties as assigned
