Service Coordinator
Immediate full-time (40 hours per week) opening at West Valley Towers, a 97-unit older-adult residential community in Van Nuys, CA.
Responsibilities
- Evaluate residents' needs to identify services that enhance quality of life and promote independence.
- Establish and maintain strong connections with local service providers, facilitating access to social, health, and wellness resources.
- Create and sustain educational health and wellness programs tailored to residents’ needs.
- Develop and maintain a comprehensive directory of community resources and services available to residents.
- Act as a resident advocate, empowering residents to advocate for themselves and liaising between residents and community services.
- Build and maintain positive relationships with community agencies, networking with service providers, and identifying new services for residents.
- Foster effective communication between residents and service providers, ensuring an open and supportive environment.
- Serve as an advocate and liaison with management, other departments, and external agencies providing social services, health benefits, training, and more.
- Provide information, referrals, and support for residents in need of crisis intervention and conflict resolution.
- Track and monitor resident referrals to external agencies, ensuring follow-up and service delivery.
- Maintain accurate and up-to-date records, including daily and monthly reports, to document service coordination and resident interactions.
- Perform other tasks as assigned to support the mission and goals of RHF.
Requirements
- Strong understanding of available supportive services and community resources for older adults.
- Proven ability to advocate for residents' needs and resolve issues.
- Commitment to maintaining high ethical standards and client confidentiality.
- Ability to work effectively with residents, staff, community partners, and service providers.
- Capable of working independently and with minimal supervision.
- Excellent verbal and written communication skills.
- Strong organizational and time management skills, ensuring timely completion of tasks and resident needs.
- Proficiency in MS Office Suite, including Word, Excel, PowerPoint, and Outlook.
Qualifications
- Bachelor’s degree in Gerontology, Human Services, Social Work, Psychology, Sociology, or a related field (preferred).
- Candidates with relevant work experience will be considered.
- Experience working with diverse groups of seniors and disabled individuals (preferred).
Pay
The anticipated base pay range for this non-exempt position is $25.00 per hour. Actual base salary considers several factors including geography, job-related knowledge, experience, and budget. The role may be eligible for overtime pay in accordance with federal and state law.
