Service Coordinator

Duties

• facilitate team meetings
• write service agreements
• provide advocacy
• act as a community liaison
• develop natural supports
• coordinate referrals/connections to other programs within LRCS

Requirements

• Knowledge of the Lakes Region
• Ability to self-manage
• Excellent communication (both written and oral)
• Time management and organizational skills
• Qualified candidates must have a Bachelor’s degree in a related field
• Prior experience with developmental disabilities
• Knowledge of IEPs and 504 Plans
• Typing and computer skills
• A valid driver’s license
• Reliable vehicle and automobile insurance

Salary

$26.00 per hour
