About the role
Join our multidisciplinary team as a Transplant Social Worker, supporting kidney transplant recipients and living donors throughout their journey.
Responsibilities
- Providing advanced psychosocial assessments and applying complex social work practices in line with ethical guidelines and legal requirements.
- Serving as part of an interdisciplinary team to address social, emotional, and economic concerns of patients, families, and caregivers.
- Utilizing age-appropriate care protocols for diverse patient populations, from neonatal to geriatric.
- Offering specialized counseling and brief therapy, and potentially diagnosing patients if properly licensed.
- Supporting patients in managing illness impacts and facilitating timely care planning.
Qualifications
- A Master’s degree in Social Work from an accredited program.
- Licensure in good standing (LMSW or LCSW) in the applicable state.
- Healthcare or mental health hospital experience preferred.
Skills
- Expert critical thinking and in-depth psychosocial assessment skills.
- Ability to work collaboratively within a team and provide complex case management.
- Knowledge of psychosocial and economic issues related to illness.
- Familiarity with age-related care protocols across patient populations.
