Senior Integrated Clinic Pharmacist

Responsibilities

• Evaluate pharmacotherapy for high-risk patients using evidence-based medicine and clinical judgment
• Execute clinical protocols as permitted by state regulations
• Optimize drug coverage benefits for cost-effective care
• Collaborate with healthcare professionals to enhance patient care
• Participate in multi-disciplinary rounds to assess patient progress
• Provide patient counseling on medication use and health management
• Engage in quality improvement initiatives for medication safety

Qualifications

• Active and unrestricted Florida pharmacist license
• Willingness to obtain additional state licenses for market support
• Pharm.D. or other advanced pharmacy degree
• 5+ years of experience as a practicing pharmacist
• Experience in a team environment supporting common goals
• Bilingual in English and Spanish, fluently reading/writing/speaking
