Part-time position, 36 hours per pay period at the Senior Enrichment Center.

Requirements

  • Current Illinois RN License
  • CPR Certification
  • Crisis De-escalation Certification and/or related training strongly encouraged

Qualifications

  • Experience in gerontological care required
  • Experience in acute-care clinical settings required
  • Psychiatric nursing experience preferred
  • Knowledge of community resources required
  • Microsoft knowledge/Computer proficiency required
  • Experience with Epic EMR preferred

Responsibilities

  • Assess and monitor patients’ health status and physical care needs at admission and during their stay, including handling physical/psychiatric emergencies
  • Perform comprehensive nursing assessments, take physician’s orders, monitor/reconcile medications, and provide health education for clients and families
  • Function as a member of the Multidisciplinary Treatment Team, performing administrative, clerical, and other tasks according to program policy
  • Demonstrate professionalism in all aspects of performance, including communication with patients, families, and regulatory agencies
  • Perform comprehensive nursing admission assessments, including chief complaints, physical/surgical history, and medication reviews
  • Update and review patient medication lists, noting allergies and responses to medications
  • Assist in developing nursing portions of Multi-Disciplinary Treatment Plans, including documentation and implementation of nursing interventions
  • Consult with Medical Director, Program Director, Therapists, and other healthcare providers to coordinate patient care
  • Attend Treatment Team meetings to discuss and update Nursing Treatment Plan sections and patient progress
  • Initiate and conduct individual and group nursing education sessions as needed or requested
  • Monitor patient consents and provide education on diagnosis, medications, and treatment processes
  • Communicate relevant clinical and patient care information to the Program Director, Medical Director, and Treatment Team members
  • Monitor patients on an ongoing basis to identify health needs, problems, or symptoms
  • Provide prompt, efficient, and responsive patient care, including coordination during medical or psychiatric emergencies
  • Complete nursing check-ins with patients each treatment day
  • Monitor, observe, and document patient behaviors and symptoms
  • Provide patient, family, and caregiver education on health-related topics
  • Assist patients with personal care needs as required
  • Participate in Quality Assurance and Performance Improvement (QAPI) efforts, supporting the Just Culture concept to improve patient safety

  • Pushing or pulling, twisting, reaching above shoulder level, reaching forward and low
  • Fine finger dexterity
  • Lifting or carrying up to 25 lbs regularly
  • Ability to hear ordinary conversation
  • Close and distance vision, color and peripheral vision

Employee may be exposed to patients with infections, communicable diseases, and aggressive behaviors.

Pay

$22.22 - $32.00 per hour
