Senior Account Manager – Home Health

Health Link Home Health & Hospice Milpitas / Santa Clara County, CA Full-Time | $45–$50/hour + Performance-Based Bonus The Role For over 20 years, Health Link Home Health & Hospice has served patients throughout the San Francisco Bay Area. We are seeking an experienced, results-driven Senior Account Manager to join our Community Relations team and support continued growth throughout Santa Clara County. Working closely with the Director of Business Development, the Senior Account Manager will focus on developing high-potential referral accounts, expanding existing partnerships, and building relationships with healthcare decision-makers throughout the South Bay. This is a field-based role for an experienced healthcare sales professional with a demonstrated ability to develop new business and produce results. The ideal candidate can work with a high level of independence, identify opportunities within the market, and build meaningful referral relationships from the ground up. You will be supported with a company vehicle and AI-powered technology designed to provide referral insights and support efficient territory routing. What You’ll Do Independently identify, develop, and grow high-potential referral accounts throughout Santa Clara CountyEstablish new relationships with healthcare decision-makers and convert those relationships into referral opportunitiesBuild and expand relationships with hospitals, skilled nursing facilities, physicians, medical groups, clinics, senior living communities, and other healthcare organizationsStrengthen existing accounts by developing additional relationships with physicians, departments, and key decision-makersIdentify opportunities to increase referrals and admissions from existing accountsConduct in-person meetings, presentations, account visits, and strategic follow-up with referral partnersEducate healthcare partners on Health Link’s home health services, clinical programs, and capabilitiesExecute business development initiatives and territory priorities in partnership with the Director of Business DevelopmentIdentify emerging market opportunities, referral trends, and competitive activity and provide meaningful feedback to the Director of Business DevelopmentMaintain strong relationships and consistent communication with key referral partnersCollaborate with Intake, Clinical, Operations, and other internal teams to support a strong referral and patient experienceMeet established referral, admission, and account growth goalsRepresent Health Link professionally throughout the South Bay healthcare community What You’ll Need 3+ years of healthcare sales, business development, account management, community relations, or related experienceDemonstrated success developing new business and growing healthcare referral relationshipsProven track record of meeting or exceeding referral, account, or growth goalsAbility to independently identify opportunities, develop new accounts, and grow existing businessStrong relationships with healthcare professionals and decision-makers in the South Bay highly valuedStrong relationship-building, communication, presentation, negotiation, and follow-up skillsSelf-motivated and comfortable working independently in the fieldStrong organizational and time-management skillsKnowledge of the Santa Clara County healthcare market strongly preferredHome health, hospice, post-acute care, senior care, hospital, or skilled nursing experience preferredValid California driver’s license and driving record that meets company requirements Benefits Performance-based bonus opportunityCompany vehicleAI-powered technology to support referral insights and efficient territory routingMedical, dental, vision, and life insurance401(k) with company matchingPaid time off
