Screening Assessment and Support Services (SASS), and Specialized Family Support Program (SFSP) Case Manager

About the role

The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for a Screening Assessment and Support Services (SASS), and Specialized Family Support Program (SFSP) Case Manager who demonstrates this mission and wants to work for an organization that makes a difference in the community.

Responsibilities

• Provides engagement, support and referrals to families/children/parents/guardians who do not, otherwise, engage in Pathways to Success Services
• Works with SASS enrolled youth and families and is responsible for the management and coordination of all SASS service delivery and case coordination activities.
• Implementation and documentation of SASS activities, and services provided.
• Works with SFSP enrolled youth and families and is responsible for the management and coordination of all SFSP service delivery and case coordination activities.
• Implementation and documentation of SFSP activities, and services provided.
• Works with FSP enrolled youth and families and is responsible for the management and coordination of all FSP service delivery and case coordination activities.
• Implementation and documentation of FSP activities, and services provided.
• Work scheduled hours and be flexible to meet client and program needs, as assigned by Supervisor or Director.
• Responsible to provide independent program coverage when scheduled.
