Job Overview

Duties and Responsibilities

• Provides skilled nursing as ordered on the plan of treatment and nursing care plan.
• Identifies and follows through on new problems, changes, and/or resolution of problems, notifying Director/Supervisor and Physician as necessary.
• Documents all actions in clinical notes, supplemental physical orders, and nursing care plan.
• Performs aseptic techniques as required.
• Administers specific treatments and medications as per care plan.
• Functions as a team member under the direction of the registered nurse.
• Communicates skilled observations and significant changes in patient status to the Director of Clinical Services and/or Clinical Manager.
• Requests modifications to orders or additional orders based on patient's clinical status and skilled nursing judgment.
• Participates in Agency's other mandated activities as requested by Agency Director.
• Adheres to MGA policies, procedures, and Handbook.
• Completes clinical documentation accurately and submits it timely per agency policy/guidelines.
• Represents the company and community positively and professionally.
• Adheres to dress code and maintains a clean, neat professional appearance.
• Reports on time and completes work within designated time.

Requirement

• Graduation from an approved certification/program/accredited school/undergraduate/graduate school or post graduate education.
• Valid/unencumbered verification/license to practice the job as described in the state where services are provided.
• Professional experience as required per state regulations.
• Successful completion of assigned screening/competency testing with a proficiency score of 80% or greater.
• Reliable transportation.
• Current TB results or Chest x-ray.
• CPR certification as per agency policy.
• Flexibility, organization, and ability to exercise independent and sound judgment.

Physical Requirements

• Sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull, and reach.
• Lighting, acoustics, air quality, temperature, and environment hazards not predictable.
• Occasionally lifts and/or moves up to 15 pounds and occasionally lifts or moves 100 pounds or more if applicable.
• Ability to perform the anticipated level of physical assistance required to provide safe client care.

Work Environment

• Highly variable work environment, generally providing care in private homes.
• Exposure to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals.
• Travel frequently to complete home health visits.
